This form enrolls one dancer in a 4-Class Package with Bollywood Dance Central — pricing shown below by location — renewed automatically as a subscription every four consecutive classes. Enrolling two children? Complete this form once per child. Already a BDC family? Use the same email and your dancer joins your existing portal.
Liability
I/we realize that participation in dance classes and activities could involve some possible personal injury. Despite precautions, accidents and injuries may occur. By signing this release form, I/we (the dancer and parent/guardian) assume all risks related to the use of any and all spaces used by Bollywood Dance Central. I/we agree to release and hold harmless Bollywood Dance Central including its teachers, dancers, staff members, and facilities used by both entities from any cause of action, claims, or demands now and in the future. I/we will not hold Bollywood Dance Central liable for any personal injury or any personal property damage, which may occur on the premises before, during or after classes. Furthermore, I/we agree to obey the class and facility rules and take full responsibility for my/our behavior in addition to any damage I/we may cause to the facilities utilized by Bollywood Dance Central. I understand that Bollywood Dance Central is a licensed and insured business. In the event that I/we should observe any unsafe conduct or conditions before, during or after my/our classes, I/we agree to report the unsafe conduct or conditions to the Owner, Director, Instructor or Staff member as soon as possible.
Photo/Video Release Waiver
I acknowledge and consent to allow Bollywood Dance Central to use: photos and/or videos of my child/myself in publications (such as flyers/brochures), advertisements (such as newspapers or online ads), our studio's website, or on social media sites (such as the studio's Facebook or Instagram account). I understand that in some cases, my child and/or my name may be used. I further understand no financial compensation will be for use of these photos/videos.
4-Class Package
I understand that a 4 class package covers 4 hours of class time. Each hour counts as an individual class. If I attend multiple hours in one day, it will count as multiple classes. I also understand that I am signing up for a subscription of 4 classes that will renew when I complete my 4 hours of consecutive class. I understand if I opt in for performances, extra hours of class time may be required and performance days will count as a class. If you miss a class, your class will not be credited forward. The only option will be a virtual makeup class! We offer one virtual makeup class every 4 classes.
Automatic Renewal
I understand that this subscription will be automatically renewed as a subscription every four classes. Reminder: if you miss a class, we can't credit that class forward (we can only offer a virtual makeup class), it will still count as a class. I understand if I want to cancel my package, I must inform Bollywood Dance Central before the 4th class date via email or my Family Portal.
